Position Overview:
We are seeking a skilled and motivated Epic Healthy Planet Analyst to join our dynamic healthcare organization. As an Epic Healthy Planet Analyst, you will play a pivotal role in optimizing and enhancing our healthcare services by leveraging the Epic Healthy Planet module. Your expertise will directly contribute to the improvement of patient outcomes, care coordination, and overall population health management.

Key Responsibilities:

Collaborate with cross-functional teams, including clinicians, IT professionals, and administrators, to analyze and understand the organization's healthcare operations, population health goals, and data requirements.
Configure, customize, and maintain the Epic Healthy Planet module to align with the organization's strategic initiatives and population health objectives.
Design and implement workflows, protocols, and interventions within the Epic Healthy Planet module to support preventive care, chronic disease management, and wellness programs.
Develop and execute data extraction, transformation, and loading (ETL) processes to integrate data from various sources into the Epic Healthy Planet module for comprehensive population health analytics.
Collaborate with data analysts and data scientists to create meaningful and actionable insights from population health data, identifying trends, gaps, and opportunities for improvement.
Monitor the performance of the Epic Healthy Planet module, troubleshoot issues, and provide timely support to end-users, ensuring the module's smooth operation and user satisfaction.
Lead training sessions and workshops for end-users, empowering them to effectively navigate and utilize the Epic Healthy Planet module to achieve population health goals.
Stay current with industry best practices, regulatory requirements, and advancements in population health management, incorporating relevant insights into module configurations and workflows.
Participate in regular meetings and contribute to the development and execution of the organization's population health strategy.
Qualifications:

Bachelor's degree in healthcare administration, information technology, or a related field. Master's degree preferred.
Epic certification in Healthy Planet strongly desired.
Proven experience (5 years) working with the Epic Healthy Planet module, including configuration, customization, and optimization.
Solid understanding of healthcare operations, population health management, care coordination, and preventive care initiatives.
Proficiency in data analysis, including the ability to extract, transform, and analyze healthcare data to drive actionable insights.
Strong project management skills, with the ability to manage multiple tasks and priorities simultaneously.
Excellent communication skills, both verbal and written, with the ability to translate technical concepts to non-technical stakeholders.
Collaborative mindset and the ability to work effectively within interdisciplinary teams.
Problem-solving attitude and the capacity to troubleshoot and resolve technical issues related to the Epic Healthy Planet module.
Knowledge of healthcare regulations, privacy laws, and security practices related to patient health information.
